6d5ed6fdd8 fix(code-editor): harden media preview against SVG XSS and improve a11y
Withhold the open-in-new-tab action for SVG previews. The link is a
top-level navigation to a blob URL, which inherits the app's origin, so
a user-controlled SVG containing <script> would execute as same-origin
script. Inline <img> rendering is unaffected and stays available.

Also give the icon-only header actions (open-in-new-tab, fullscreen
toggle, close) explicit aria-labels and mark their decorative SVG icons
aria-hidden, so screen readers announce each action instead of relying
on title alone.

de13eed72a fix(file-viewer): revoke blob URL created by an aborted media load
If the file switches while the fetch/PDF-validation is awaiting, the effect
cleanup runs before objectUrl is assigned, so it revokes nothing and the
in-flight load then creates a blob URL that is never freed. Check
controller.signal.aborted right after createObjectURL and revoke + bail so
rapid switching can't leak object URLs (CodeRabbit: Stability, Major).

e39de299c3 fix(file-viewer): prevent stale media preview when switching files
Gate the rendered blob on a per-source key (projectId:path:kind) and clear
the URL in the missing-projectId branch, so a previous file's blob can no
longer flash under the new filename and the error state fully replaces an old
preview (CodeRabbit: Functional Correctness, Minor).

92b5b935fd fix(file-viewer): address review feedback on media preview
- Never write preview-only or binary files: handleSave is a no-op when
  previewKind/isBinary, and the text buffer is cleared when switching to a
  media/binary file, so Cmd/Ctrl+S can no longer post a stale or empty buffer
  over the file on disk (CodeRabbit: Data Integrity, Critical).
- Choose the fallback MIME type per file extension (single source of truth in
  previewableFile.ts) instead of per preview-kind, and only override when the
  server Content-Type is missing or generic, so webm/ogv/ogg/flac/svg render
  correctly (CodeRabbit: Functional Correctness, Major).
- Harden the PDF iframe: validate the %PDF- magic bytes and pin the blob MIME
  to application/pdf so a mislabeled HTML/SVG file can't execute in the app
  origin. A sandbox attribute is intentionally not used because Chromium's
  built-in PDF viewer will not load inside any sandboxed frame (CodeRabbit:
  Security, Critical).

1df336ca2d feat(file-viewer): preview media files inline instead of showing "binary file"
Natively renderable files (images, PDFs, audio, video) opened from the
Files tab previously fell back to the generic "binary file" placeholder.
Classify previewable extensions and render a new CodeEditorMediaPreview
component that streams the bytes through the existing authenticated content
endpoint and displays them with the right native element (<img>, <iframe>,
<video controls>, <audio controls>) via a blob: URL. Non-previewable
binaries (zip, exe, avi, mkv, fonts, ...) still show the binary message.
No backend or dependency changes.

* fix(chat): prevent chat interface crash when AskUserQuestion payload is malformed

Loading a session that contains an AskUserQuestion tool call could crash the
entire chat interface with "TypeError: e.map is not a function".

The AskUserQuestion tool is configured with `defaultOpen: true`, so
QuestionAnswerContent renders as soon as the session loads. Its array guard
(`!questions || questions.length === 0`) only checked for truthiness, and
`q.options` was mapped/iterated with no guard at all. When `questions` or
`options` arrive from the session transcript as a non-array value, the
`.map()` / `.some()` calls throw and take down the whole chat view via the
error boundary.

Guard both with `Array.isArray()` so a single malformed message can no longer
crash the interface.
